What is Liposculpture?

Liposculpture, also known as lipofilling, is a technique that allows adding volume to specific areas of the body using the patient’s own fat (autologous fat) as a natural filling material.

In addition to volumetrically enhancing the recipient areas, liposculpture also improves the body contour by aspirating localized fat deposits (from donor areas) that are resistant to diet and exercise.

It can also be used to correct irregularities or asymmetries in areas such as the thighs, flanks, or abdomen.

This technique, which has been used for over 20 years, provides excellent, stable, and long-lasting aesthetic and reconstructive results. Autologous fat is considered the best and safest filling material, as it is made from the patient’s own tissue.

The use of artificial or synthetic materials, on the other hand, can sometimes cause complications such as extrusion or foreign body reactions, which may require removal.

How is it performed?

Liposculpture is a meticulous and personalized procedure performed in two main stages, allowing the body to be reshaped in a harmonious and natural way:

1. Liposuction of fat from the donor area

To obtain fat for later implantation, the donor areas – such as the abdomen, inner or outer thighs, flanks, or other regions – are liposuctioned.

During this liposuction, performed under general anesthesia, a fine cannula connected to an aspiration device is used. The collected material is separated into different layers:

  1. blood, anesthetic fluids, and liquid fat on one side;
  2. pure fat on the other, which will later be used for reinjection.

2. Fat infiltration into the recipient area

The purified fat obtained is injected using a fine microcannula, either subcutaneously or intramuscularly, through tiny incisions near the target areas.

In surgical liposculpture, the recipient areas – the parts of the body that receive the fat extracted from other regions – vary according to the patient’s aesthetic goals. However, the most common recipient areas include the buttocks, breasts, and face (such as the cheeks).

Usually, a single treatment session is sufficient to achieve the desired result.

Who is this surgery for?

Liposculpture is intended for individuals who wish to reshape their body by removing localized fat deposits in areas such as the abdomen, flanks, thighs, and back, while simultaneously increasing volume in other areas, such as the buttocks, through fat transfer. This procedure is recommended for people who are within a healthy weight range, have good skin elasticity, and do not have medical conditions that could complicate the surgery.

Liposculpture is especially suitable for those who struggle to eliminate localized fat through diet and exercise, but it is not a weight-loss solution.

After the fat is implanted, the treated area must be immobilized for about 10 to 14 days using compression garments.

There will always be some residual swelling in the implanted area, which may take several days to subside. However, patients can usually resume normal daily activities within 4 to 5 days, while physical exercise and sun exposure should be avoided for about one month.

What is the scar like after Liposculpture?

Liposculpture generally results in minimal scarring, as the incisions are very small and strategically placed to minimize visibility, serving only as entry points for the cannulas.

Like any surgical procedure, liposculpture involves risks that should be carefully considered before deciding to proceed.
Some of the associated risks, although rare, may include:

  • Adverse reactions to anesthesia;

  • Infection at the incision site;

  • Skin irregularities;

  • Changes in sensitivity;

  • Healing problems;

  • Risk of asymmetry.

Liposculpture is not recommended for individuals with serious medical conditions such as uncontrolled heart disease, uncontrolled diabetes, or other severe systemic disorders.
Patients with untreated psychological conditions, such as eating disorders or severe depression, should avoid undergoing surgery.
It is also recommended that patients who plan to become pregnant postpone the procedure until after childbirth.
Before surgery, you will have the opportunity to discuss your medical history, lifestyle, and expectations with the plastic surgeon to ensure you are fully informed and prepared.

Most of the transferred fat remains stable and long-lasting. However, a small percentage may be reabsorbed by the body within the first few weeks. Maintaining a stable body weight is essential to preserve the results over time.

Liposuction removes localized fat, while liposculpture goes a step further – it uses part of that fat to fill and contour other areas, achieving more balanced and natural results.

Dr. Francisco Martins de Carvalho is a specialist in Plastic, Reconstructive and Aesthetic Surgery. He completed his degree in Medicine at the School of Medicine of the University of Oporto and the Internship in Specific Training in Plastic, Reconstructive and Aesthetic Surgery took place at Hospital de São José (Lisbon) and Hospital de São João (Oporto).

Dr. Francisco Martins de Carvalho performed several internships, namely on Oncology Surgery of the Head and Neck at the Portuguese Institute of Oncology – Oporto and a fellowship of Hand Surgery, organized by the Portuguese Society of Hand Surgery. He also performed international internships in Aesthetic Surgery, Breast Reconstruction and Lymphedema at the Clinic Planas – Barcelona, ​​at the University Hospital of Brussels and at the Villa Sant’Apollonia Clinic – Bergamo, having contacted with references in Plastic Surgery such as Dr. Jaume Masià, Dr. Moustapha Hamdi and Dr. Enrico Robotti.

Currently, he is also a plastic surgeon at the Hospital de São João – Oporto, and a Invited Lecturer at the School of Medicine of the University of Oporto.
